This form must be completed in its entirety before The Village Condominiums, owned by Broadway Civic Center Properties, will accept a Credit or Debit payment of any kind over the phone.

Whether or not any Credit or Debit Card information has been provided by resident, no payments will be accepted without the completion of this authorization form or completion of the Lease Addendum for Automated Electronic Payment of Rent, etc.

This form can only be used to authorize one transaction and will not authorize automatic monthly payments. If you would like to learn more information about setting up automated monthly rent payments with your Credit or Debit card, please contact the office.

Please fill out the form below carefully as any incorrect information may result in the return of your rent payment, or your rent payment may be put on hold for verification of authorization.
